Under the Health Information Act (HIA), this form allows you to authorize another individual to communicate with our clinic staff regarding your personal medical information. Filling out this form authorizes us to share details related to your health care with the person(s) you name, such as a family member or caregiver.

Household members
Family Linking Form
This form is intended for patients of Brilz Associate Clinic who have other family members in their household who are also patients at the clinic. By completing the Family Linking form, you provide the names and birthdates of those family members so that the clinic can “link” your files. Linking records allows the clinic staff to update demographic information across the entire family when changes are made.

This agreement is intended to ensure the safe and responsible use of opioid medications as part of your treatment plan. If opioid therapy is deemed necessary, this form outlines your responsibilities as a patient, including proper use, storage, and the importance of not sharing medications. It also covers potential risks, regular follow-ups, and conditions under which the medication may be discontinued.

Wainwright Prenatal Shared Care Program

With the changes occurring in healthcare, many rural communities no longer provide obstetrical care, and as a result, patients must travel to receive prenatal care. In Wainwright, obstetrical physicians have come together to provide obstetrical care to Wainwright and the surrounding areas.
The increase in maternity patients has made it more challenging for obstetrical physicians, who are required to be on call 24/7 for their prenatal patients. This inhibits their ability to be out of town and have uninterrupted free time, and it requires them to find coverage whenever they need to be away from work. To put it simply, it keeps them from having a life. This change allows these physicians to continue providing optimal care to prenatal patients.
What does this mean for you?
The Obstetrical Physician Group in Wainwright will now operate from a single location. Prenatal care will be provided at the Wainwright Primary Care Network.

The clinic will run on Tuesdays and Wednesdays. Patients can phone the PCN on weekdays to book their appointments at (780) 842-6676.

By integrating new tools into our workflows, we aim to support our physicians in providing timely, high-quality care while streamlining daily operations within the clinic.
AI Scribe
Our physicians have begun using an AI tool called AI Scribe to assist with medical charting. This tool helps reduce the amount of time spent documenting appointments, allowing doctors to focus more on direct patient care and other important aspects of your visit.
How It Works
AI Scribe transcribes in-clinic conversations between the physician and the patient in real time. It does not record the interaction, nor does it provide any medical advice or decision-making. It simply creates a written summary to assist the doctor in completing accurate notes efficiently.
Your Choice Matters
If you prefer that AI Scribe not be used during your appointment, please inform your physician at the beginning of your visit. We respect your preferences and are happy to accommodate your choice regarding this tool.
